Your first primary care visit: build a useful visit folder

Start before you leave home

At Dr. Elsaadi’s office, your first appointment is arranged by phone. That conversation is a good time to say whether you are transferring care, recently left a hospital or have records at another practice. Ask what the office needs before your visit; a stack of papers handed over at check-in may not include the information needed most.

Give the team the previous office or laboratory name and approximate dates. Ask how permission to request records should be handled. Do not assume that every health system shares the same chart.

Make three simple sections

Use an envelope or folder you already have. The purpose is to make information easy to find, not to create a perfect medical history.

• Front: photo ID and insurance card. Write down the plan, network or medical group questions you want answered.
• Middle: all current medications, plus a note about what you actually take and anything you recently stopped or could not obtain.
• Back: records the office requested and a short list of other clinicians, recent tests and upcoming appointments.

Choose the question you do not want to miss

Write one sentence beginning, “The main thing I want help with today is…” Follow it with when the concern began and how it affects a normal day. This is more useful than trying to remember every detail once you are in the exam room.

You can bring other questions too. At the start, ask which can be addressed today and which need their own follow-up. Before leaving, check that you understand the next step, who will contact you about results and whether another appointment is needed.

Make the arrival easier

The office is at 2015 N Waterman Ave, Suite A, San Bernardino. Ask about parking, the entrance and any accessibility needs when scheduling. Dr. Elsaadi speaks English, Spanish and Arabic; office staff speaks English and Spanish.

Once your first visit is complete, provide an email address to receive your Healow invitation. There is no portal account to create before establishing care.
