Call the office with your discharge instructions to arrange the recommended follow-up. Do not wait for every record to arrive before asking about scheduling.
Use the discharge plan as your starting point
Discharge instructions can include changes in medications, follow-up visits, care at home and problems to watch for. Keep that plan together with the hospital contact information. Follow the instructions you were given, including whom to call if a problem develops.
When you call Dr. Elsaadi’s office, have the hospital name, discharge date and recommended follow-up timing ready. If you cannot find the timing or instructions conflict, say so. This guide cannot determine the right interval for your hospitalization.
Make four labeled sections
A folder with clear labels can help both you and anyone assisting you.
- Medicines: the discharge list, current bottles and any differences you need clarified.
- Appointments: recommended visits, dates already arranged and visits still to schedule.
- Results: completed reports you have and tests you were told were still pending.
- Home needs: instructions about equipment, services or help with daily care.
Assign an owner to unfinished tasks
For each pending result or appointment, ask who is responsible and how you should expect to hear back. “Someone will call” is hard to follow up on if you do not know which office.
Keep hospital and specialist instructions with their dates. If a previous medication list differs from the discharge list, do not create your own combination; contact the responsible clinician or pharmacist for clarification. Tell the office about any difficulty obtaining the prescribed medicines.
Keep continuity realistic
Dr. Elsaadi is affiliated with St. Bernardine Medical Center and Community Hospital of San Bernardino and may personally follow patients in those facilities when needed. That does not mean every hospitalization is managed by him or that all records automatically reach the office.
Bring your paperwork and ask what additional records should be requested. For an urgent concern, follow your discharge instructions and call for medical help; for an emergency, call 911. Do not rely on a portal appointment request for immediate attention.
