Multiple specialists, changing medicines and outside tests can leave unclear responsibilities. Bring the latest instructions and ask who is following each issue. A clear record of what remains unfinished can be as useful as a list of diagnoses.
After a hospital stay
Call the office with your discharge date and the recommended follow-up timing. Keep the medication instructions and pending-test information together. Do not assume records have already arrived simply because both offices use electronic systems.
Dr. Elsaadi is affiliated with Dignity Health – St. Bernardine Medical Center and Community Hospital of San Bernardino. He may personally follow patients there when needed; arrangements depend on the hospitalization.

Dr. Elsaadi has practiced in the Inland Empire since 1991. His independent practice offers adult primary care with room to discuss how different recommendations fit your circumstances. Ask when a specialist is needed and how the offices will communicate.
If a family member helps with your care, tell the office how you would like them involved. Ask about authorized communication and portal access rather than sharing a password.
